- 1.Two passport-style photographs
- 2.Copy of your government-issued identity document with photograph
- 3.Copy of your birth certificate (if unavailable: other evidence of birth such as church, school or medical records, plus proof it is unavailable) (optional)
- 4.Inspection and admission, or inspection and parole, documentation (unless applying under INA 245(i))
- 5.Copy of the I-797 approval or receipt notice for the Form I-130 filed for you (unless filing it together with this application)
- 6.Form I-693 medical examination and vaccination record, or partial I-693 (if applicable; must be submitted with the I-485) (optional)
- 7.Certified police and court records of all criminal charges, arrests or convictions, regardless of outcome (optional)
- 8.Documentation of past or present J-1 or J-2 status, including proof of compliance with or a waiver of the 2-year foreign residence requirement (optional)
- 9.If adjusting from a K (fiancé(e)) visa: evidence you married your U.S. citizen petitioner within 90 days of admission (optional)
